On the issuance of a license for the conduct of the type of economic activity

In accordance with Article 14 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The license fee is paid by the licensee no later than ten working days from the date of making a record of the decision to issue a license in the license register of the Ministry of Internal Affairs. The
document confirming the payment of the license fee is a copy of the receipt issued by the bank, a copy of the payment power of attorney with a bank note, a receipt from a payment terminal, a receipt (check) from the post office of communication. Non-payment of the license fee in accordance with paragraph 3 of the second part of Article 16 of the Law is the basis for deciding to suspend the license in whole or in part. In view of the foregoing and in accordance
with Article 6 of the Law and the Resolution of the Cabinet of Ministers of Ukraine dated August 05, 2015 No. 609 “On approval of the list of licensing bodies and recognition some decrees of the Cabinet of Ministers of Ukraine that have lapsed into force”

I ORDER:

1. To issue to the LIMITED LIABILITY COMPANY “TOP TRADING LINE” (identification code 43082920; location: Shevchenko street, house 24 A, Kharkiv city, 61013) a license to conduct business activities for the production and sale of personal protective equipment. Place of the specified type of activity at the address: selo Sosnivka, Lviv district, Lviv region.
