On the issuance of a license for conducting business activities and the
place of conducting business activities

In accordance with Article 14 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The fee for issuing a license is made by the licensee no later than ten working days from the date of making a record of the decision to issue a license in the license register of the Ministry of Internal Affairs. The

document confirming the payment of the fee for the license is a copy of the receipt issued by the bank, a copy of the payment power of attorney with a bank note, a receipt from a payment terminal, a receipt (check) from the post office of communication. Non-payment of the fee for issuing a license in accordance with paragraph 3 of part two of Article 16 of the Law is the basis for deciding to suspend the license in whole or in part. In view of the foregoing and in accordance

with Article 6 of the Law and the Resolution of the Cabinet of Ministers of Ukraine dated August 05, 2015 No. 609 “On approval of the list of licensing bodies and recognition some decrees of the Cabinet of Ministers of Ukraine that have lapsed into force”

I ORDER:

1. To issue to the LIMITED LIABILITY COMPANY “CAROUSEL” (identification code 41976470; location: Verbova Street, building 17-A, Kyiv city, 04073) a license to conduct business activities for the production and sale of special personal protective equipment. Place of activity: Pasichna Street, building 127, Lviv city.

2. According to the information submitted by the limited liability company “OCHOTA” (identification code 41688009; location: Liagina Street, house 35/2, Nikolaev city, 54020), note the store at the address: Volodymyr Velykogo Street, house 13, Ivano-Frankivsk city, — the place of the proceedings business activities in the trade of non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 millimeters and a bullet flight speed of more than 100 meters per second; sale of special vehicles loaded substances of tear and irritant action, personal protection, active defense.

3. Department of Communications (Andreev D.) to ensure that the order is posted on the official website of the Ministry of Internal Affairs for publication no later than the working day following the day of adoption of this decision.
