Issuance of the certificate is carried out in paper form (A4 format) without the use of special forms.
To obtain the above certificate, a citizen can apply with a corresponding application, regardless of the registration of the place of residence, to the territorial units of the State Tax Service of Ukraine.
For the record! The Unified State Register is conducted for the purpose of identifying a person for registration, issuance, exchange, transfer, removal, return to the state, recognition as invalid and destruction of the documents provided for by this Law.
