Migrant workers explained how to get an identity card for return to Ukraine
We inform citizens that the identity card for return to Ukraine (hereinafter - the certificate) is issued to citizens of Ukraine, as well as to persons recognized by Ukraine as stateless persons, or stateless persons permanently residing in Ukraine, forcibly relocated to the territory of a state recognized by Ukraine as an aggressor state or state occupier, in whose name, using the means of the Unified State Demographic Register (hereinafter - the Register), a citizen's passport in the form of a card or a passport of a citizen of Ukraine for traveling abroad, a residence permit for permanent residence, or a temporary residence permit, or in respect of which a decision has been made to recognize a stateless person or information is available on the registration of a passport of a citizen of Ukraine, which is recorded in the form of electronic data in the departmental information system of the State Tax Service, as well as persons under the age of 18, in respect of whom there is no information in the Register DMS or departmental DMS system.
Registration of the specified identity card (without its presence) is carried out by the DMS departments on the basis of an appeal:
- one of the parents or other legal representative of a person under the age of 18;
- one parent, spouse, adult child, relative (adult, minor) sibling;
- official appeal to the Ministry of Integration.
To obtain a certificate, a legal representative, a family member submits the following documents to the DMS department at the place of application:
- a written application indicating information about the person in whose name the certificate is issued (last name, first name, patronymic), including variables, date of birth, place of birth, unique registration number of the Register (if available), registered/declared place of residence, as well as circumstances that cause the need for registration the certificate, and the method of further transfer of the certificate to such person;
- a birth certificate or birth registration document issued by the competent authorities of a foreign state and legalized in the prescribed manner, unless otherwise provided for by an international treaty of Ukraine, consent to the obligation of which is provided by the Verkhovna Rada of Ukraine (including a copy thereof certified in the prescribed manner), or e-birth certificate, or extract from the State Register of Birth - in the case of registration of an identity card under the age of 18, in respect of which there is no information in the Register or the departmental system of the DMS. In the absence of a birth certificate, such information is entered by the DMS employee on the basis of the information specified in the written statement, which is subsequently checked by the DMS employee;
- a document certifying the identity of a legal representative, a family member (identity document confirming the citizenship of Ukraine, or an identity document confirming its special status, e-passport, e-passport for traveling abroad, e-permit for temporary residence, e-certificate for permanent residence, iDocument), and a document confirming the authority of the person (in the case of submission of documents by a legal representative) or the fact of family relations (in the case of submission of documents by a family member). In the event that legal representatives, family members do not have documents confirming their powers or the fact of family relations, such information is entered by the DMS employee on the basis of the information specified in the written statement, which is subsequently checked by the DMS employee according to the State Register of Civil Status Acts of Citizens;
- two color photographs of a person under the age of 18, in respect of which there is no information in the Register or the departmental system DMS, 3.5x4.5 centimeters (for pasting into the certificate) and 10x15 centimeters in size (for making a digitized image of the person's face by scanning to the departmental DMS system) - in the case of registration of an identity card under the age of 18, in respect of which there is no information in the Register or the departmental system of the State Tax Service.
