On the issuance of licenses for conducting business activities, expanding the
type of conducting business activities

In accordance with Article 14 and the second paragraph of the ninth part of Article 15 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The fee for issuing a license is made by the licensee no later than ten working days from the date of making a record of the decision to issue a license in the license register of the Ministry of Internal Affairs.

The document confirming the payment of the fee for issuing a license is a copy of the receipt issued by the bank, a copy of the payment order with the bank's mark, a receipt from the payment terminal, a receipt (check) from the post office of communication. Non-payment of the fee for issuing a license in accordance with paragraph 3 of part two of Article 16 of the Law is the basis for deciding to suspend the license in whole or in part.

In view of the foregoing and in accordance with Article 6 of the Law and Resolution of the Cabinet of Ministers of Ukraine of August 05, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. Issue:

NATURAL PERSON — ENTREPRENEUR CHERNETSIY OLEKSIY VOLODYMYROVYCH (place of residence: Sichovykh Striltsiv Street, building 6 B, apartment 45, Stryi city, Lviv region, 82400, registration number of the taxpayer's account card and other mandatory payments 2686819430) license to conduct economic activities for the repair of firearms non-military use, pneumatic weapons with a caliber of more than 4.5 millimeters and a bullet flight speed of more than 100 meters per second, trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons with a caliber of more than 4.5 millimeters and a bullet flight speed of more than 100 meters per second; sale of special equipment charged with tear and irritant substances, personal protection, active defense. STATE SCIENTIFIC RESEARCH INSTITUTE OF CHEMICAL PRODUCTS (identification code 14015318; location: Sadovyi bulvar street, building 59, Shostka city, Sumy region, 41100) license for conducting economic activities for the production of ammunition for non-military firearms and trade in ammunition for firearms non-military weapons. Place of activity: building No. 6 (shop), building No. 8 A (production facility), Sadovyi Bulvar street, house 59, Shostka city, Sumy region. Details for payment of license fee: recipient of GUK in Sumy region/Shostkinska MTG/22011800, recipient code 37970404, recipient's bank Treasury of Ukraine (EAP), account number (IBAN) UA048999980334199896070018536, budget income classification code tu 22011800).

2. Expand PRIVATE ENTERPRISE “PHOBOS” (identification code 32313503) at the address: Velyka Berdichevskaya street, house 13, Zhytomyr city, conducting business activities for repairing non-military firearms, trade in non-military firearms, cold weapons, pneumatic weapons with a caliber of more than 4.5 millimeters and a bullet flight speed of more than 100 meters per second; sale of special equipment charged with tear and irritant substances, personal protection, active defense as an additional part, namely activities for the production of ammunition for non-military firearms. 3. Department of Communications (Shevchenko A.) to ensure that the order is posted on the official website of the Ministry of Internal Affairs for publication no later than the working day following the day of adoption of this decision.
