THE ORDER

On the place of conducting economic activities, expanding the conduct of economic activities

In accordance with Article 14, paragraph 2 of the ninth part of Article 15 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The fee for issuing a license is made by the licensee no later than ten working days from the date of making a record of the decision to issue a license in the license register of the Ministry of Internal Affairs.

The document confirming the payment of the fee for issuing a license is a copy of the receipt issued by the bank, a copy of the payment order with the bank's mark, a receipt from the payment terminal, a receipt (check) from the post office of communication. Non-payment of the fee for issuing a license in accordance with paragraph 3 of part two of Article 16 of the Law is the basis for deciding to suspend the license in whole or in part.

In view of the foregoing and in accordance with Article 6 of the Law and Resolution of the Cabinet of Ministers of Ukraine of August 05, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. According to the information submitted by an individual — entrepreneur BURLACHUK IRYNA GEORGIIVNA (place of residence: Dovzhenko Street, house 9 B, apartment 152, Ivano-Frankivsk city, 76005, registration number of the taxpayer's registration card and other mandatory payments 2279024681), consider the store at the address: Belvederska Street, house 39, Ivano-Frankivsk city, is the place of economic activity on trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 millimeters and bullet flight speed of more than 100 meters per second; sale of special means charged with tear and irritant substances, personal protection, active defense.

2. Expand the limited liability company “FLARM” (identification code 40007855; location: Olesya Honchara Street, building 74 B, apartment 22, Kyiv city, 01054) conducting business activities for the production of non-military firearms and active defense equipment as an additional part, namely activities with trade in non-military firearms, ammunition for it, sale of special means of active defense. Place of activity: Nagirna Street, house 12, Kyiv city.
