On the issuance of licenses for the right to conduct certain types of economic
activities and the place of conduct of activities

In accordance with the Law of Ukraine “On Licensing of Business Activities” and the Resolution of the Cabinet of Ministers of Ukraine of August 05, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. Issue:

1) an individual — entrepreneur Reznik Vitaly Alexandrovich, who lives at the address: ul. Ivana Pulyuya, 33-A, sq. 804, m. Lviv, 79060, — registration number of the taxpayer's account card and other mandatory payments 2238306833, — license to carry out activities for the production of ammunition for non-military firearms, trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons weapons with a caliber of more than 4.5 mm and a bullet flight speed of more than 100 m/s. Place of activity at: ul. Kulparkivska, 226, row X, garage № 21, m. Lviv;

2) to the limited liability company “EXIM EQUIPMENT” (str. Yaroslaviv Val, 13/2-B, m. Kyiv, 01054, identification code 40145358) is a license for conducting business activities on trade in non-military firearms, ammunition for it. Place of activity at: ul. Privokzalnaya, 50, m. Boryspil, Kyiv region.

2. According to the information submitted by an individual - entrepreneur Yampol Alexander Pavlovich, who lives at the address: ul. Striletska, 3, sq. 3, m. Vinnytsia, 21000, — registration number of the taxpayer's account card and other mandatory payments 2067116154, consider the store at the address: ul. Kyivska, 78, m. Vinnytsia is a place of economic activity with trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 mm and bullet flight speed of more than 100 m/s; sale of special equipment charged with tear and irritant substances, individual protection, active defense.

3. Control over the execution of the order is entrusted to the Licensing Office (Kamyshanov V.I.).
