On the issuance and cancellation of licenses for the right to conduct certain types of economic activities

In accordance with the Law of Ukraine “On Licensing of Business Activities” and the Resolution of the Cabinet of Ministers of Ukraine of August 05, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. Issue:

1) an individual — entrepreneur Okhrimenko Oleksandr Viktorovich who lives at the address: ul. Kozacha, 1, sq. 18, m. Zaporozhye, 69095, — registration number of the taxpayer's account card and other mandatory payments 2742019998, — license to carry out activities for repair, trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 mm and speed of flight bullets over 100 m/s; sale of special means charged with tear and irritant substances, personal protection, active defense. Place of activity at the address: ul. Sedova, 36, m. Zaporozhye;

2) an individual — entrepreneur Strelitsa Volodymyr Dmytrovych, who lives at the address: ul. Molodizhna, 9, Bilmak village, Zaporizhzhya region, 71001, — registration number of the taxpayer's account card and other mandatory payments 1981303657, — license for conducting activities on trade in non-military firearms, ammunition for it, cold weapons, caliber pneumatic weapons more than 4.5 mm and a bullet flight speed of more than 100 m/s; sale of special means charged with tear and irritant substances, personal protection, active defense. Place of activity at the address: Sobornyi lane, 30, Bilmak village, Zaporizhzhya region;

2) an individual — entrepreneur Zoya Leonidovna Golovko, who lives at the address: ul. Svyato-Troitskaya, 6-A, p. Snovsk, Chernihiv region, 15200, — registration number of the taxpayer's account card and other mandatory payments 3102516486, — license to conduct activities on trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4,5 mm and flight speed bullets over 100 m/s; sale of special means charged with tear and irritant substances, personal protection, active defense.

2. In connection with the submission of a copy of the death certificate of an individual — entrepreneur Vasyukov Andriy Nikolaevich, who lived at the address: ul. Gagarina, 11, sq. 111, m. Melitopol, Zaporizhzhya region, 72316, — registration number of the taxpayer's account card and other mandatory payments 2701217912, — cancel the license of the AE series No. 292986, issued in accordance with the order of the Ministry of Internal Affairs of 10.03.2015 No. 248 for the conduct of economic activities for the repair of non-military firearms, trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons with a caliber of more than 4.5 mm and a bullet flight speed of more than 100 m/s.

3. Department of Communications (Shevchenko A.V.) ensure the posting and publication of this order on the official website of the Ministry of Internal Affairs.

4. Control over the execution of the order is entrusted to the Licensing Office (Kamyshanov V.I.).
