THE ORDER

On the issuance of licenses for the conduct of business activities and

place of activity

In accordance with Article 14 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The fee for issuing a license is paid by the licensee no later than ten working days from the date of publication on the official website of the Ministry of Internal Affairs of the decision to issue a license.

Licensees confirm the payment of the fee for issuing a license by submitting to the Ministry of Internal Affairs the relevant document, which is a copy of the receipt issued by the bank, a copy of the payment order with the bank's mark, a receipt from the payment terminal, a receipt (check) issued by the post office of communication. Non-payment of the fee for issuing a license in accordance with paragraph 10 of part two of Article 16 of the Law is the basis for deciding to cancel the license.

In view of the foregoing and in accordance with Article 6 of the Law and Resolution of the Cabinet of Ministers of Ukraine of August 05, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. Issue to the LIMITED LIABILITY COMPANY “SARA ARMS” (French Boulevard, 27, m. Odessa, 65044, identification code 41016902) license for carrying out activities for the production of active defense equipment.

Details for payment of a fee for issuing a license: recipient of the UC in the city. Odessa/Primorsky district/22011800; recipient code 38016923; GUDKSU beneficiary bank in Odessa region; bank code 828011; account number 33211896770008; classification code 22011800.

2. According to the information submitted by the individual-entrepreneur CHOVBAN ALEXANDER YURIYOVICH, who lives at the address: ul. Kamenskogo, 132, m. Mukachevo, Zakarpattia region, 89600, registration number of the taxpayer's account card and other mandatory payments 2920002790, consider the store at the address: ul. Gorky, 25, Teresva village, Tyachiv district, Transcarpathian region, place of economic activity with trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 mm and bullet flight speed of more than 100 m/s; sale of special equipment, charged with substances of tearing and irritating action, personal protection, active defense.

3. Department of Communication (Shevchenko A. B.) ensure the publication of the order on the official website of the Ministry of Internal Affairs following the day of the decision of the working day.
