On the issuance of licenses for proceedings

place of activity

In accordance with Article 14 of the Law of Ukraine “On Licensing of Business Activities” (hereinafter — the Law), a one-time fee in the amount of one subsistence minimum is charged for issuing a license, based on the size of the subsistence minimum for able-bodied persons, which is valid on the day of the decision to issue a license. The fee for issuing a license is paid by the licensee no later than ten working days from the date of publication on the official website of the Ministry of Internal Affairs of the decision to issue a license.

Licensees confirm the payment of the fee for issuing a license by submitting to the Ministry of Internal Affairs the relevant document, which is a copy of the receipt issued by the bank, a copy of the payment order with the bank's mark, a receipt from the payment terminal, a receipt (check) issued by the post office of communication. Non-payment of the fee for issuing a license in accordance with paragraph 10 of part two of Article 16 of the Law is the basis for deciding to cancel the license.

In view of the foregoing and in accordance with Article 6 of the Law and Resolution of the Cabinet of Ministers of Ukraine of August 5, 2015 No. 609 “On Approval of the List of Licensing Bodies and Recognition as Invalid Certain Decisions of the Cabinet of Ministers of Ukraine”

I ORDER:

1. To issue to the individual entrepreneur VITRUK DMITRY IVANOVICH, who lives at the address: ul. Chervonogvardeyskaya, 55, Komysh-Zorya village, Bilmatsky district, Zaporizhzhya region, 71030, — registration number of the taxpayer's account card and other mandatory payments 2997416518, license to conduct activities for trade in non-military firearms, ammunition for it. Place of activity at the address: Sobornyi lane, 30, Kuibysheve village, Zaporizhzhya region.

Details for the payment of the license fee: UC recipient in Kuibyshev district/Kuibyshev district/22011800; recipient code 37892243; GUDKSU recipient bank in Zaporizhzhya region; bank code 813015; account number 33213896770152; classification code 22011800.

2. According to the information submitted by the HUNTING AND FISHING TRADE AND PRODUCTION ENTERPRISE (ul. Stepova, bud. 58, m. Odessa, 65005, identification code 22448853), consider the store at the address: ul. Hrubnik, building 20, Dobroslav village, Limansky district, Odessa region, is the place of economic activity on trade in non-military firearms, ammunition for it, cold weapons, pneumatic weapons of caliber over 4.5 mm and bullet flight speed of more than 100 m/s; sale of special means charged with substances of tear and irritant action, personal protection, active defense.

3. The Department of Communications (Shevchenko A.V.) to ensure the publication of the order on the official website of the Ministry of Internal Affairs following the day of the decision of the working day.

4th. The Licensing Administration (Chernyshova O.V.) to ensure control over the execution of this order.
