In accordance with the requirements of the Laws of Ukraine “On State Statistics” and to comply with the orders of the Ministry of Health of Ukraine dated 10.07.2007 No. 378 “On approval of health reporting forms and instructions for filling them out”, registered in the Ministry of Justice of Ukraine on September 03, 2007 under No. 1009/14267, dated 05.03.2013 No. 180 “On approval of primary forms accounting documentation and reporting on the issues of monitoring the epidemic situation of HIV infection and instructions for filling them out”, registered with the Ministry of Justice of Ukraine on March 27, 2013 under No. 495/23027, and in order to optimize the reporting of health care institutions of the Ministry of Internal Affairs of Ukraine

I ORDER:

Approve the List of forms of sectoral statistical reporting of health care institutions of the Ministry of Internal Affairs of Ukraine (attached).
Consider that the order of the Ministry of Internal Affairs of Ukraine dated October 08, 2001 No. 870 has ceased to be effective.
Control over the execution of the order will be entrusted to the Deputy Minister — Head of Staff Chebotar S.I.
